3. Mechanism of Action (18.)

Amoxicillin inhibits bacterial cell wall synthesis. Clarithromycin binds to the 50S subunit of the 70S ribosome to inhibit protein synthesis. Bonoprazan does not require acid activation; it inhibits H⁺,K⁺-ATPase in a reversible, potassium-competitive manner. It remains in the acid-secreting regions of parietal cells for an extended period, raising the pH in the stomach and thereby enhancing the activity of both antibiotics against Helicobacter pylori. Bonoprazan itself has no anti-Helicobacter pylori or urease-inhibiting activity.

4. Indications and Usage

Eradication of Helicobacter pylori infection (gastric ulcers, duodenal ulcers, gastric MALT lymphoma, idiopathic thrombocytopenic purpura, the stomach following endoscopic treatment for early gastric cancer, Helicobacter pylori gastritis, etc., according to the indications in the original Japanese version).

Adults typically take this medication twice daily, consuming the entire contents of the daily blister pack each time, for 7 consecutive days. Pack 400: Clarithromycin 200 mg per dose; Pack 800: Clarithromycin 400 mg per dose. Amoxicillin 750 mg per dose; Takecab 20 mg per dose.
