It is unknown whether Mounjaro causes thyroid C-cell tumors in humans, including medullary thyroid carcinoma (MTC), as the relevance of tirzepatide-induced thyroid C-cell tumors in rodents to humans has not been established. This drug is contraindicated in patients with a personal or family history of MTC or in patients with multiple endocrine neoplasia type 2 (MEN 2). Inform patients of the potential risk of MTC and the symptoms of thyroid tumors (e.g., neck lump, difficulty swallowing, shortness of breath, persistent hoarseness). The value of routine monitoring of serum calcitonin or thyroid ultrasound for the early detection of MTC in patients receiving Mounjaro is uncertain. Such monitoring may increase the risk of unnecessary procedures, as serum calcitonin testing has low specificity and a high prevalence of underlying thyroid disease. A significantly elevated serum calcitonin level may suggest MTC; calcitonin levels in MTC patients are typically > 50 ng\/L. If an elevation is detected, further evaluation is warranted; patients with thyroid nodules identified on physical examination or neck imaging should also undergo further evaluation.<\/p>\n
2. No studies have been conducted in patients with a history of pancreatitis. It is unknown whether patients with a history of pancreatitis are at increased risk of pancreatitis when using Mounjaro. After initiating treatment, carefully monitor for symptoms of pancreatitis (including persistent severe abdominal pain, sometimes radiating to the back, with or without vomiting).
